Tribal Certificate

Tribal Certificate form can be obtained from  DC Office,Lawngtlai Facilitation Centre.

Please read the form carefully and fill it up with a readable handwriting

Documents to be attached on the form:

• One Passport-size photo 
• Attested Photocopy of Voter ID (Parents Voters ID photocopy for minors) attested by A gazetted officer .
• A consent letter for Local Council/Village Council that you are Schedule Tribe
• A consent letter for President/Secretary that you are Schedule Tribe
